ADMISSION REQUIREMENT

For Incoming Freshmen Students

  1. Original form 138-a or the senior high school report card

  2. Certificate of Good Moral Character from the last school attended

  3. 2 pcs 2×2 ID picture, 2 pcs 1×1 ID picture (any background color)

  4. Original and photocopy of PSA Birth Certificate

  5. 1 long brown envelope

For Transferees

  1. Certificate of Transfer Credentials/Honorable Dismissal

  2. 2 pcs 2×2 ID picture, 2 pcs 1×1 ID picture (any background color)

  3. Original and photocopy of PSA Birth Certificate

  4. Certificate of Good Moral Character from the last school attended

  5. Transcript of Records for evaluation

  6. 1 long brown envelope

ADDITIONAL REQUIREMENTS

  1. For Criminology students only. Drug Test and Laboratory Result ( HBSG, CHEST X-RAY)

  2. For Midwifery students only. Medical Examination ( Urinalysis, CBC, Chest X-ray, HB Sag and Pregnancy, Fecolysis)

  3. For 4p’s beneficiary only; Municipal Link Certification and Certificate of Income

  4. For IP’S only; Certificate of IP

  5. For PWD only ID

  6. For 4P’s, IP’s, nor PWD; Certificate of Indigency

ENROLLMENT PROCEDURE

Step 1: Guidance Office

  • Entrance Examination

Step 2: Office of the Student Affairs

  • Orientation

Step 3: Program Head

  • Evaluation/Loads/Subject

Step 4: Office of the Registrar

  • Submit/check documents/interview/encoding of subjects/release of enrollment form.
